What Is Acne Conglobata?

Acne Conglobata is a rare but very severe form of nodulocystic acne. It causes deep, painful lumps and cysts that connect under the skin. This condition is chronic (long-lasting) and often leads to severe scarring and skin disfigurement.

It usually affects:

Face

Back

Chest

Shoulders

Upper arms

Buttocks

and thighs

It is more common in young adult men and is rarely seen in older adults.

Symptoms

Common symptoms include:

Large painful nodules (deep lumps)

Interconnected blackheads (often in groups of three)

Deep abscesses filled with foul-smelling pus

Oozing or draining skin lesions

Severe scarring

Skin crusting after drainage

In very severe cases (called Acne Fulminans):

Fever

Weight loss

Joint pain

Severe inflammation

Many patients also suffer from depression and anxiety due to scarring.

Causes

Acne conglobata happens due to several factors:

1. Hormones

High levels of androgens (male hormones like testosterone) increase oil (sebum) production.

2. Bacteria

A skin bacteria called Cutibacterium acnes contributes to inflammation.

3. Follicle Blockage

Dead skin cells and oil block hair follicles, causing deep inflammation.

4. Triggers

Anabolic steroid use (common in bodybuilders)

Sudden stopping of hormones

Androgen-producing tumors

Thyroid medications

Certain genetic conditions

Treatment

Acne conglobata requires aggressive medical treatment.

1. Main Treatment (First Choice)

Oral isotretinoin (retinoid) for 20–28 weeks or longer.

2. Steroids

Oral prednisone may be given at the beginning to reduce severe inflammation.

3. Antibiotics

Doxycycline

Minocycline

(Used especially if there is infection.)

4. Other Options

DapsoneTNF-alpha inhibitors (in resistant cases)

Laser therapy

Surgery for large nodules

Scar treatment after healing

⚠️ Isotretinoin must NOT be used during pregnancy.

Dermatologist supervision is necessary.

Prevention

There is no guaranteed prevention, but risk can be reduced by:

Avoiding anabolic steroids

Not using hormonal drugs without medical advice

Early treatment of severe acneGood skin hygiene

Avoid excessive heat and sweating on affected areas

Early treatment helps prevent permanent scarring.

Important Note

Acne conglobata is not ordinary acne. It is severe, painful, and can cause permanent skin damage and emotional distress. If someone has deep painful nodules with drainage and scarring, they should see a dermatologist immediately.
